Our Aged Care Services comprise of federally funded subsidised programs including; a specialist residential aged care, community aged care (Home Care); The Assistance with Care and Housing for the Aged (ACHA).

James Watson Hostel — Residential Aged Care

The James Watson Hostel is a specialized residential aged care service, providing 24/7 supported accommodation for 40 homeless, or at risk of homelessness, men.

James Watson Hostel is accredited under the Aged Care Act 1997 by the Australian Aged Care Quality Agency.

The specialized homeless services for vulnerable older people include:

  • Single, fully furnished, reverse cycle air-conditioned, ensuited rooms.
  • Twenty four hour care including individual care planning and personal care.
  • On site access site to a multidisciplinary primary health and allied health personnel.
  • Individual and group lifestyle and activity programs.
  • Nutritious home cooked meals reviewed regularly by a dietician.
  • Hospitality services include on site laundry and cleaning services
  • Qualified and skilled staff experienced in managing challenging behaviours.

Conditions of Entry

  • An Aged Care Client Record (ACCR) is completed by the Aged Care Assessment Team (ACAT) referring the person for residential aged care, either on respite or as a permanent entry.
  • Being homeless, at risk of homelessness, poor and/or prematurely aged.

St Bart’s Home Care

St Bart’s Home Care is a federally funded, approved provider which supports older people as they require and determine, to remain living independently in their own homes.

Objective

To be of service to older people in their own homes, through providing information, resources, linkages and services as required.

Conditions of Entry

People wishing to access St Bart’s Home Care program:

  • Require an Aged Care Assessment , through My Aged Care.
  • Be on a fixed low income
  • Be willing to pay a nominal fee for service.

Assistance with Care and Housing for the Aged Program (ACHA)

To assist people over 50, on a fixed low income, who may need help to stay living in your home or provide assistance if you do not have a home.

Objective

To advocate, link and support vulnerable older people to find a home or remain living in their own home.

Condition of Entry

People wishing to access St Bart’s  ACHA program

  • Be over 50 years of age.
  • Require assistance with finding a home or
  • require advocacy and or linkages to support services to remain living in their own home.
  • On a fixed low income.
  • Consent is provided.

Either self or referral from an agency for the prematurely aged, over 50 years, frail, low income and in insecure accommodation or homeless.
