Since 1963, St Bart’s has been standing by the side of Western Australians experiencing homelessness, mental health challenges, trauma and hardship.

When we first started, St Bart’s was a small homeless shelter for men, we are now providing support to more than 530 vulnerable people through a range of services.

About the role

Our Assistance with Care and Housing (ACH) program assists people in the Metropolitan area to access appropriate long-term accommodation and support and is specifically tailored to help those who are over 50 years of age and low income, over 45 for Aboriginal and Torres Strait Islander people, living in insecure housing and experiencing homelessness. 

The Team Leader ACH is responsible for managing the ACH program on a day to day basis, overseeing the Case Managers and responsible for a small caseload of consumers with complex needs. 

Reporting to the Aged Care Services Manager, you will ensure the ACH service is monitored and coordinated in line with policies and procedures and ensure the continuous improvement of service outcomes. 

Taking into consideration the qualifications, skills and experience of the successful applicant the base salary offered for this position spans from $84,849.44 to $88,702.64 plus superannuation. 

About you

To be successful in this role you will need a tertiary qualification (minimum Diploma) in Community Services, Aged Care or a similar discipline together with demonstrated work experience. 

In addition, our ideal candidate:

  • Is a compassionate person with great interpersonal skills, verbal and written communication skills
  • Has proven leadership skills and experience
  • Has advanced knowledge of Aged Care Systems and legislation in relation to Commonwealth Home Support Program

As we work with vulnerable people, the successful candidate will be required to show proof of influenza and COVID-19 vaccinations unless a medical exemption is provided. The successful candidate must also be willing to obtain an NDIS Worker Screening Check, have a current First Aid Certificate and a current WA “C” Class Drivers Licence.
