About the role

As a Case Manager you will work in the Independent Living Program (ILP) providing case management and personalised support to consumers experiencing mental health challenges on their recovery journey, assisting them with personal skills development to sustain and maintain their own homes.

You will engage with our consumers to develop individual recovery plans that are person centred, address identified needs and goals, as well as work closely with stakeholders assisting consumers in developing their independent living skills and supporting them to live in the community.

This is a permanent part-time (32 hours per week) opportunity working Monday to Friday, based in East Perth.

Taking into consideration the qualifications, skills and experience of the successful applicant the base salary offered for this position spans from $82,044 – $88,288 per annum plus superannuation pro-rata to hours worked.

About you

To be successful in this role you will need a tertiary qualification (minimum a certificate IV) in Social Science, Community Services or similar discipline together with relevant demonstrated work experience in the mental health sector and/or homelessness and trauma informed practices

In addition, our ideal candidate has:

  • Highly developed communication and interpersonal skills, with the ability to build rapport with consumers, their families, and other networks.
  • Well-developed assessment and planning skills with the ability to adapt to change.
  • A ‘can do’ attitude

  • A current satisfactory National Police Certificate
  • A First Aid Certificate
  • A current WA “C” Class Drivers Licence
